Our disability services and day programs are available across the country. We support people through the NDIS, as well as people who are not NDIS eligible. It’s your choice which supports you receive – we’ll tailor our services to meet your needs.

Specialist Support Coordination 

Specialist Support Coordinators aim to reduce complexity in the person’s support environments in the context of broader systems of support, whilst also assisting the person to connect with NDIS supports, negotiate solutions with multiple stakeholders and build capacity and resilience.
Specialist Support Coordination is delivered utilising a specialised approach, necessitated by high and complex needs or high-level risks in a person’s situation. We address highly complex barriers impacting on your ability to implement your plan and will assist you to ‘optimise’ your plan ensuring that you are getting the most out of your funded supports.
Includes:

  • Assisting with managing a budget for each type of support and negotiating prices and service agreements
  • Coordinate complex support and services which are developed and delivered within the funding plan
  • Develop a support plan that identifies clear actions
  • Ensure support and service is developed and delivered according to the wishes of each participant and their families
  • Research and recommend service providers to suit your needs
  •  Experienced in mental health, intellectual disability, torture and trauma issues, positive behaviour support and restrictive practices
  • Assisting participants to achieve goals through building capacity, knowledge, resourcefulness and confidence.
  • Ongoing support from a social worker
  • Maintaining effective transparent relationships and communication with internal and external providers and allied health care professionals as well as government agencies relevant to your need
  • Assist you in planning ahead to prepare for your plan review.
  • We are obliged to mange conflict of interest; we will recommend service providers most suite your need.

Aged Care

Are you over 65 years? Two out of five people with disability are 65 or older, whereas almost nine in 10 people aged 90 and over have a disability.

If you become a participant of National Disability Insurance Scheme (NDIS) before you turn 65 years old, Choice Disability Services can assist you to continue to receive services by the scheme. We can assist you to access the following services – improve functional abilities, education in behaviours, support and team-based resources that maximise your independence under Continuity of Support (CoS) program.

Everyday living skills

We can help you build your routine skills by partnering in various skills, activities, and tasks in the community or even in the comfort of your home. Our support services, including experimenting with new things and supporting you in whatever activities you want to do to build or enhance the skills you need in your daily living.

Our services will be customised to suit indoor tasks such as Household chores, Personal care, health and fitness, and outdoor activities, including Buying groceries, paying bills, and joining community programs. Our main aim is to help you develop employable skills, achieve your desires, and attain independence while enjoying every bit of this lifestyle- based support service.

High intensity support

Our highly skilled team of professionals are committed to providing you with the best possible support. We understand that clients have varying needs that require different approaches, and so we tailor our supports for both long-term and short-term care.

Besides ensuring that your day-to-day routine is manageable and achievable, we want to ensure that you get time for family and friends. Instead of struggling alone with high intensity personal activities, you can get support either in your home or at the community level. Our support supervision and care include personal activities such as assistance with injections, feeding and management, wound management, tracheotomy care, and bowel care.

Supported independent living

Our supports are always accessible 24/7, depending on your needs. We understand that every client needs different levels of care. We can make plans for you to share convenient accommodation and support with others.

Besides living with other people, you can be on your own in shared accommodation and still access the best supports from our dedicated teams. Health and wellbeing

We understand that therapeutic care requires specialised training and attention to your needs or even for those you care about, if you are ordering the service for someone else. And so, we train our staff to ensure that those supporting you have knowledge of spinal cord injuries, positive behaviour, dysphagia, diabetes, mental health, epilepsy, and general health promotion.
